If something here is wrong

Email corrections@careonrecord.com with the page URL and what is inaccurate. We respond within five business days.

Because every flag is drawn from a public source, a correction usually means the underlying record changed. We will re-check the source, update the page, and tell you the outcome. Where the source itself is wrong, we will link to the issuing agency's own correction process.

Where our data comes from

• NPPES NPI Registryprovider identity, taxonomy, addresses, deactivations
• CMS Doctors and CliniciansMedicare participation and assignment
• Florida Department of Healthlicense status and disciplinary history
• Florida Agency for Health Care Administrationfacility licensure. Inspection deficiencies are not included: AHCA publishes those only as per-facility PDFs, and we do not have them yet.
